Schedule an Intake Call

During the call our Intake Coordinator will review your information, verify your insurance and co-pay amount, recommend which therapist may be a good fit for you and go over scheduling options.

You will receive a phone call at the scheduled Intake Call time. Please be in a private space during this 15 minute phone call as I may be asking you questions regarding therapy.

What Happens Next?

1) schedule an initial APPOINTMENT

After speaking with our Intake Coordinator, we will schedule an initial Intake appointment based on your availability. Please note we will require a credit/debit card on file and a $1.00 service fee to hold your first appointment when scheduling an intake appointment.

2) complete Your intake forms

Abundance Therapy Center is a paperless practice. After scheduling your appointment, our intake coordinator will email you a link to log into your client portal and complete the intake forms. Please fill out the intake form at least 48 hours before your first appointment on any tablet, smartphone or computer. If your intake forms are not completed in time, your appointment will not be held. Please note, the form takes about 10-15 minutes to complete. Though we have a computer at our offices, admin staff aren’t always on location and your therapist may be in another session prior to yours. Waiting to complete the intake form may cut into your initial session time, we strongly advise doing so ahead of time. Sessions cannot start without the intake form being completed.

If needed, clients can access a Release of Information form for a letter writing or records request.

3) Meet with your therapist

During your intake appointment, your therapist will ask you a variety of questions to better understand why you are seeking therapy, determine if it is a good fit, set goals, and schedule your next appointment. This is also the opportunity for you to ask any questions you might have and determine if this is the right therapist for you.

No Show & Late Cancellation Policy: Once you schedule your appointment, we are reserving the hour for you. Any cancellation or missed appointments without at least 48- hours notice will be subject to a $120 fee. Insurance companies do not pay charges for missed appointments. Please be mindful of our therapist time and give us at least a 48-hours notice if you need to cancel or reschedule your appointment. You can do so by contacting the office or responding to the confirmation (for intake appointment) or contacting your therapist directly (for ongoing weekly sessions).
